A Licensed Vocational Nurse (LVN) who, under the supervision of the Clinical Manager, coordinates the timely and accurate flow of clinical information during the referral process. The Home Health Intake Liaison serves as a key point of contact between referral sources, patients, and the agency. This role is responsible for supporting the intake process, coordinating referrals, verifying eligibility, and building strong relationships with hospitals, physicians, and community partners to support census growth and smooth patient transitions into home health services.PIH Health is a nonprofit, regional healthcare network that serves approximately 3 million residents in the Los Angeles County, Orange County and San Gabriel Valley region.
