Handle all general inquiry calls and emails from potential participants and referral sources. Gather information from potential participants to complete detailed intake form. Submit Medical Treatment Plan to participant’s medical physician and follow-up.\n2. Conduct in-person and virtual presentation outreach in the community.\n3. Assist participants in applying and renewing San Francisco Paratransit group van transportation service.\n4. Assist and maintain participant’s Child and Adult Care Food Program (CACFP) eligibility annually. Maintain and input daily meal record.\n5. Maintain participant’s Adult Day Service participant agreement.\n6. Provides administrative support to the Director, including preparation of statistical reports and office administration.\n7. Performs other duties as assigned.\nQualifications:\n1. High school diploma with a minimum of two years of office experience; Associate Degree is preferred.\n2. Basic skills and knowledge of Microsoft office processing (Word, PowerPoint, Excel) in English and Chinese.\n3. Excellent organizational, interpersonal, verbal and written communication skills.\n4. Bilingual in English and Cantonese.\n5. Must have TB clearance within 12 months prior to employment and maintain clearance annually thereafter.\n6.
