In this pivotal role, you will work in partnership with and under the direction of the Care Coordination Program Director to support the effective delivery of Care Coordination Wraparound services across all program locations.\nServing as a lead team member and key support to the Program Director, you will provide guidance, mentoring, training, and operational assistance to Care Coordinators while helping ensure quality service delivery, regulatory compliance, and program effectiveness.
