Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by the certification. The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan and possess the ability to assess data reflective of the patient's status. Interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs. Provide care needed for adult and geriatric patient groups. Job Functions: Ensures resident's Activities of Daily Living (ADLs) needs are met as outlined in the plan of care and in accordance with policies and procedures. Includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care, etc. Documentation is completed per protocol. Maintains resident's environment and safety as outlined in the plan of care. This includes answering call lights and alarms promptly, provision of safety mats and low beds, and restraint use under the direction of Nursing. Maintains cleanliness of the unit, behavior and fall prevention, and appropriate use of and monitoring malfunctioning equipment. Documentation is completed per protocol. Ensures resident's nutritional needs are met as outlined in the plan of care. Includes meal delivery, tray preparation, adaptive equipment, feeding, and nourishments. Honors resident's preferences and choices. Documentation is completed per protocol. Meets resident's rehabilitative (restorative and maintenance) needs as outlined in the plan of care. Includes range of motion, ambulation, transfers, proper body alignment, and use of assistive devices. Documentation is completed per protocol. Reports resident's rights, refusals, and concerns to Charge and/or Supervising Nurse in a timely fashion in accordance with policies and procedures. Identifies changes in resident's physical, emotional, and psychosocial status and reports to Charge Nurse promptly. Occupational exposure to blood borne pathogens is reasonably anticipated.
