The Access and Care Navigator manages wait lists across Network 9 programs and service locations, coordinates admissions with clinical teams, and promotes timely, person-centered access to care.\n\nSalary: $45,000-$47,500\n\nHours: Variable schedule\n\nLocation: Joliet, IL\n\nREPORTS TO:\n\nDirector of Counseling and Specialty Services\n\nNETWORK OR DEPARTMENT:\n\nNetwork 9\n\nESSENTIAL FUNCTIONS:\n\nRespond to requests for behavioral health services by conducting initial screenings and determining appropriate service needs.\nMeet with individuals seeking services as quickly as possible to complete consents, screenings, assessments, and other required intake documentation.\nCoordinate referrals and appointments across all Network 9 programs and service locations to ensure timely access to behavioral health services.\nManage centralized wait lists by monitoring program capacity, communicating with program staff, and facilitating placement into services as openings become available.\nCollaborate with clinicians, program staff, referral sources, healthcare providers, schools, hospitals, and community agencies to coordinate admissions and continuity of care.\nEducate individuals served and family members regarding available services, eligibility requirements, insurance coverage, and treatment options.\nMaintain accurate and timely documentation in accordance with organizational policies, Medicaid requirements, accreditation standards, and applicable regulations.\nParticipate in quality improvement activities designed to improve access to care, reduce wait times, and enhance the overall experience of individuals seeking behavioral health services.\n\nSKILLS AND ABILITIES:\n\nKnowledge of behavioral health assessment, screening, intake, care navigation, and referral processes.\nKnowledge of outpatient behavioral health services, crisis intervention, community resources, and levels of care.\nExcellent customer service, interpersonal, and communication skills while creating a welcoming and person-centered experience for individuals seeking services.\nAbility to establish rapport and effectively engage individuals, families, referral sources, and community partners.\nStrong organizational and time management skills with the ability to manage multiple referrals, assessments, appointments, and competing priorities.\nAbility to assess needs, exercise sound clinical judgment, and facilitate timely access to appropriate behavioral health services.\nKnowledge of Medicaid, commercial insurance, confidentiality requirements, and behavioral health documentation standards.\nProficiency with electronic health records, scheduling systems, Microsoft Office applications, and other technology used to coordinate care.\nAbility to collaborate effectively with multidisciplinary teams to support continuity of care and positive outcomes.\nDemonstrates commitment to person-centered, trauma-informed, culturally responsive, recovery-oriented, and strengths-based practices.\n\nPHYSICAL DEMANDS:\n\nRemains in a stationary position, including sitting or standing, for prolonged periods.\nOperates a computer, telephone, and other standard office equipment.\nFrequently communicates with individuals served, family members, referral sources, employees, and community partners in person, by telephone, and through virtual platforms.\nOccasionally travels between Trinity Services locations and community partner sites as needed.\nOperates a motor vehicle while conducting agency business.\nOccasionally lifts and carries materials weighing up to 25 pounds.\nOccasionally reaches, bends, stoops, kneels, and performs repetitive hand and wrist movements associated with computer use.\n\nQUALIFICATIONS:\n\nBachelor's degree in Counseling, Psychology, Social Work, Human Services, or a related behavioral health field required.\nMinimum of two years of experience in behavioral health, care coordination, intake, case management, crisis intervention, or a related human service's setting preferred.\nExperience conducting behavioral health screenings, coordinating referrals, or navigating individuals through behavioral health systems preferred.\nExperience working with electronic health records, scheduling systems, Medicaid, and commercial insurance preferred.\nknowledge of community behavioral health
