The NP will provide advanced clinical care to hospice patients, focusing on comfort, symptom management, and quality of life. This role works closely with physicians, nurses, social workers, and other care team members to ensure patient-centered care.Key ResponsibilitiesProvide comprehensive assessments, diagnosis, and treatment planning for hospice patientsManage pain and symptoms in accordance with hospice and palliative care guidelinesCollaborate with the hospice interdisciplinary team to develop and update care plansConduct routine and urgent patient visits in homes, facilities, or hospice settingsEducate patients and families on disease progression, medications, and care plansDocument all patient interactions accurately and timely in EMRParticipate in IDG (Interdisciplinary Group) meetingsEnsure compliance with all regulatory and organizational standardsQualificationsCurrent Nurse Practitioner license in the state of [Insert State]Board certification as a Nurse Practitioner (FNP, AGNP, or related)Minimum of 12 years of experience in hospice, palliative care, or related field preferredDEA license (if prescribing controlled substances)Strong clinical assessment and critical thinking skillsExcellent communication and interpersonal skillsAbility to work independently and as part of a teamPreferred QualificationsExperience in hospice or palliative careKnowledge of end-of-life care practices and regulationsBilingual (if applicable)
