Service Coordinator (LPN)

RiseWell Community Services is looking for a Licensed Practical Nurse for our Supportive Housing for Herkimer Gardens. In this role, you will be providing management and administration of medications, in conjunction with the psychiatrist, NPP, and registered nurse as a member of a supported housing facility for persons with severe and persistent mental illness. Ensure medical standards as per program regulations are being provided.QUALIFICATIONS: Currently licensed as a Licensed Practical Nurse by the New York State Education Department. A valid and clean NY State driver's license. Minimum 1 year of experience in a treatment or rehabilitation setting preferred. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.JOB FUNCTION: Provide assistance to people with chronic medical needs with accessing supports and services that will meet their individual needs. Assist residents in achieving their maximum level of functioning and independence in the least restrictive environment of their choice. Provide support for the other Case Managers and staff. Act on behalf of the supervisor in their absence.Essential FunctionsWork with staff to determine eligibility and appropriateness of applicants.Assess any medical crisis that arises and ensure linkage to any necessary medical interventions,Coordination with all providers to ensure continuity of care between providers daily,Provide on-going education and counseling to assist clients who have increasing medical concerns as needed.Assess participants’ needs and communicate with team about needed interventions and necessary follow-up daily.Provide voluntary medication assistance as needed.Provide linkages to PCPs and medical services on an ongoing basis.Inform program of hospitalization and medication changes.Participate in inpatient discharge meetings and other collaborative meetings.Assist individuals to establish and work towards their own desired life goals.Educate and assist individuals to obtain medical, social, psychosocial, financial, legal, educational, mental health and other medical services.Visit each individual assigned to assist them in meeting their established goals.Facilitate resident groups and workshops.Facilitate community trips.Assist individuals in developing support systems, which include both formal and informal supports.Empower individuals to be involved in the decision making process to help them choose the best options.Advocate on behalf of individuals with other service providers to assist them in reaching their desired goals.Assist individuals in coping with the demands of work, school and social relationships.Maintain regular contact with other service providers to ensure a full continuum of services.Conduct reassessment meetings with each individual to assess their progress at least every 6 months or when circumstances or needs change.Maintain required documentation in compliance with funding sources and agency policies.Attend supervision, staff meetings and trainings as scheduled.Report all incidents and problematic events immediately to Program Manager or designee.Provide guidance and training to Case Managers and other staff in the program.Act on behalf of Program Manager in their absence.Comply with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description.Utilize all agency based IT applications pertaining to this job description in a proficient manner.Remain current on changing developments in individual field of expertise.
