At Providence Care, we provide end of life care to patients and provide support to family members.\n\nResponsibilities:\n\nLooking for someone with a Hospice heart and passion for providing end of life care\nProvide Case Management to patients and families, this includes symptom management, medication management, and collaboration with members of the IDG\nResponsible for overseeing patient plan of care with the collaborating physician\nProvide supervisory visits and oversight for LPN and CNAs\nWill conduct start of care visits and collaborate with Care Navigators\nProvide education to patients, families, and facility staff\nProvide oversight to Home Health Aides and LPNs\n\nQualifications:\n\nMust have a RN license unincumbered in the state you are applying\nReliable transportation and valid drivers license, clean MVR\nExperience in Hospice/Home Health is preferred\n\nProvidence Care is an Equal Opportunity Employer.
