This includes assisting with daily living skills,\npersonal care, and fostering independence. These responsibilities are in alignment with the\nIndividual Support Plans (ISP) developed by the Oregon Department of Human Services. The\nDSP helps clients improve life skills such as cooking, cleaning, scheduling, shopping,\ntransportation, housing, budgeting, community involvement, and more. Accurate daily\ndocumentation of progress is required.\nRoles and Responsibilities:\nSupport in Daily Living:\no Assist with personal care, including hygiene, grooming, and daily routines.\no Implement skill-building activities as outlined in the ISP to foster independence.\no Support clients in household tasks such as cooking, cleaning, budgeting, and\nmore.\nCommunity Integration and Socialization:\no Provide transportation and accompany individuals to appointments, outings, and\nsocial activities.\no Encourage social participation and integration into the community.\nDocumentation and Reporting:\no Complete and submit daily progress notes and reports according to State and\nagency requirements.\no Communicate effectively with Grow’s management team, client/guardians, and\nother professionals apart of the client’s care team.\nEmergency Response:\no Be responsive and prepared to handle emergencies and critical situations\neffectively. This may include filling out additional documentation upon request\nsuch as a Serious Incident Report.\nGeneral Staff Qualifications:\nA staff member providing services to an individual must meet the following qualifications:\nBackground and Compliance:\no Ability to pass a background check through the Orchard Background Check Unit\nand pass periodic driving record checks.\no Must not be listed on the excluded or debarred providers list by the Office of the\nInspector General.\nCompetency and Experience:\no Competency in handling high-pressure situations.\no Experience in providing general care and support for individuals with\ndevelopmental disabilities.\no Knowledge of home care services and proficiency in supporting life skills\ndevelopment.\no Ability to understand and follow written and oral instructions and orders.\nCommunication Skills:\no Proficient in both written and oral communication.\no Able to communicate effectively with individuals, health care providers, case\nmanagers, and others involved in care.\no Expected to keep an open line of communication with Grow’s management team,\nclients, and anyone else involved in the clients’ care plan.\nEducation and Licensing:\no High school diploma or equivalent required.\no Hold a current, valid, and unrestricted professional license or certification where\napplicable.\no Current driver's license, auto insurance, and reliable transportation.\nTechnology and Tools:\no Must own and be proficient in using a smartphone for work-related tasks.\nAdditional Requirements:\no At least 18 years of age and legally eligible to work in the United States.\no Understand and maintain confidentiality of personal information and adhere to all\nprivacy standards.\no Certified in CPR and First Aid upon starting employment and maintaining\ncertification.\no All other required trainings and documents need to be kept up to date, renewed,\nand sent into management.
