CenterWell Senior Primary Care - - Responsibilities: Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care (POC); Assist with self-administered medications by opening bottle caps and reading labels; Take and document vital signs (temperature, pulse, blood pressure) and report abnormalities; Assist with personal hygiene and activities of daily living; Assist with feeding and nutrition monitoring; Assist with toileting and catheter/ostomy care as applicable; Assist with mobility, transfers, positioning, and ambulation per Plan of Care; Document interactions and care rendered to ensure continuity of care
