Job Description Occupational Therapist PositionOur organization is looking for a dedicated and compassionate Occupational Therapist (OT) to become a part of our Healthcare & Medical Services team in Grand Junction, CO. This role is centered around delivering exceptional, evidence-based occupational therapy to adults and seniors within a post-acute, long-term care, and assisted living setting.Key ResponsibilitiesAssess residents' functional capabilities and create personalized treatment plans.Implement therapeutic interventions aimed at fostering independence, safety, and meaningful participation in daily activities.Collaborate with an interdisciplinary care team, including nursing, physical therapy, speech therapy, social services, and family members, to enhance each resident's quality of life.Conduct thorough assessments and establish measurable goals for therapy.Provide specialized treatments such as ADL retraining, adaptive equipment training, cognitive and perceptual remediation, balance and coordination training, and home safety recommendations.Document all services accurately and promptly, adhering to state, federal, and facility regulations.Educate residents and their families on home programs, energy conservation, fall prevention, and safe caregiving practices.Assist in discharge planning and transitions of care in collaboration with the care team.QualificationsProfessional, empathetic, and organized demeanor.Strong clinical reasoning abilities.Capability to customize interventions based on the individual needs and preferences of residents.Working ConditionsThis position allows for the development of meaningful relationships with residents and their families, contributing to a supportive, team-oriented atmosphere that prioritizes wellness, dignity, and resident choice.Benefits and Growth OpportunitiesAs an Occupational Therapist with us, you will play a vital role in assisting residents in achieving their highest level of functional independence and comfort, while supporting their physical, emotional, and social well-being on a daily basis.
