You’ll help guide individuals and families through healing, recovery, and ongoing care with compassion, clinical expertise, and a human-centered approach.This role is ideal for a nurse who values autonomy, connection, collaboration, and the opportunity to make a lasting difference in patients’ lives every day.The salary range for this position is $45.42 – $71.98 per hour, depending on factors such as experience, education, training, seniority, and applicable collective bargaining agreements.This position is represented by a collective bargaining agreement. Multiple openings may be available.What You’ll DoBuild trusting relationships with patients and caregivers through compassionate, individualized nursing care in the home setting.Assess patients’ physical, emotional, and environmental needs to determine appropriate home health or hospice services.Develop and manage personalized care plans in collaboration with physicians, caregivers, and interdisciplinary care teams.Provide skilled nursing care, education, and guidance that empowers patients and families to confidently manage health needs at home.Coordinate care across the continuum to ensure patients experience smooth transitions and connected support.Monitor patient progress, adjust care plans as needed, and help patients work toward meaningful health goals.Support patients and families through difficult moments with empathy, including bereavement planning and follow-up services when appropriate.Maintain accurate and timely documentation that supports high-quality, compliant patient care.Work independently while staying connected to a collaborative and mission-driven care team.What You BringA patient-first mindset and a passion for delivering compassionate care in the home setting.Strong clinical judgment and the ability to make thoughtful decisions independently.Excellent communication and relationship-building skills with patients, families, and care teams.Flexibility, organization, and comfort working in a dynamic environment.A commitment to treating every patient with dignity, respect, and empathy.What You’ll NeedGraduate of an accredited School of NursingCurrent Registered Nurse (RN) license in the state of practiceBasic Life Support (BLS) certification upon hireValid driver’s license and proof of auto insuranceMinimum of two years of acute clinical, home health, or hospice nursing experienceBasic computer proficiency, including Microsoft Office applicationsBonus QualificationsBachelor of Science in Nursing (BSN)Previous home health experience Why PeaceHealth At PeaceHealth, we care for the whole person — and that includes our caregivers.
