Networks with area community providers and provides case management supports for the benefit of the clients. Supports management in the training and supervision of direct care staff, interns, and volunteers. Performs other duties as assigned. EEO.\nESSENTIAL JOB FUNCTIONS\nProgram Management:\nSupports management in the administration of program standards and procedures related to the operation and provision of services.\nSupports management in monitoring costs and allocation of resources for program, in accordance with agency and client needs.\nSupports the management in implementing changes in the program reflecting identified agency priorities and strategic plan.\nProvides case management training and assists in assigning of caseloads. Provides appropriate case management supports to assigned case load.\nAssists in monitoring use and maintenance of the agency vehicle(s) assigned to program.\nConducts supervisory spot checks in conjunction with management.\nMaintains property to ensure building/office is welcoming upon entrance.\nWorks with staff to promote a clean, safe, and organized work environment for both staff and clients.\nCompletes monthly data submission in accordance with the Department of Mental Health and Addiction Services, and/or additional contracting agencies.\nMeets regularly with identified clients, in-home, as appropriate, to assess current status and progress.\nMaintains working relationships with identified community providers.\nAttends agency sponsored in-services, outside trainings, conferences and meetings to facilitate continued professional growth.\nClinical Services and Supervision:\nEstablishes rapport and working relationship with assigned clients.\nDevelops and writes comprehensive community service plans with client, completes progress notes as assigned and documents all communications with community providers as appropriate.\nProvides on-call coverage in order to be available to staff in the program.\nProvides crisis intervention and support as clinically indicated.\nAttends and participates in monthly staff meetings, clinical rounds, and all-treater meetings.\nParticipates in and conducts supervision as needed and identified by management.\nFacilitates/co-facilitates weekly psychoeducational groups.\nNetworks with area community providers for the benefit of the clients.\nQuality Assurance – Chart Documentation and Professional Development\nAdheres to policies and practices as outlined in the staff handbook and program operations manual.\nConsistently works in a respectful, constructive, and cooperative manner with fellow staff and providers.\nUpholds all Health Insurance Portability and Accountability Act (HIPAA) regulations and secures all necessary releases of information for outside communication.\nModels culturally competent behavior interacting with clients, professionals/providers, and fellow staff.\nMaintains client records as assigned and in accordance with contractual, licensing, and accreditation requirements.\nSupports management in the training and supervision of direct care staff for the clients on their caseload.\nAdheres to the standards of the Council on the Accreditation of Rehabilitation Facilities (CARF).\nParticipates in agency committee work when available or as permitted.\nAddresses concerns with staff or other professionals constructively and in a timely fashion.\nSchedules, attends and participates in agency meetings, team meetings, and inter-disciplinary team meetings.\nCompletes all training requirements as assigned by Agency.\nQUALIFICATIONS:\nMaster’s degree required with a minimum of three (1) years’ managerial experience in the behavioral health field. CT Licensed Professional required LMSW, LPCA or above.\nPrior experience working with people with severe and persistent mental illness, intellectual disabilities, homeless population, and/or co-occurring substance use disorder..\nValid Connecticut Driver’s license is required. Scattered site and non-24 hr. program positions require the employee to maintain reliable transportation and be willing to transport clients in personal vehicles.\nFirst Aid/CPR certification or willingness to obtain certification within 90 days of hire.
