Community Services Specialist (Social Work) - Hybrid, Lewiston

We have immediate openings for Community Services Specialists in Androscoggin and Franklin Counties. As Community Services Specialists, you will work hybrid positions, 40 hours/week, based out of either our Lewiston or Wilton offices. As a team member of the Community Services Department, responsibilities include: Providing information, assessing needs, educating about long term support options and counseling/assisting clients and families in planning for the future by making appropriate referrals and assisting in linking to services that meet the needs of older adults, persons with disabilities, families and caregivers. Serving as a collaborative liaison with community health and human service organizations. Qualifications 4-year degree in social service or related field or a 2-year Associate's Degree in Human Resources with one year of social services ex Licensed social worker or eligible to become licensed One to three years community experience in case management, professional social work, nursing, gerontology, health or other related human services preferred Ability to plan, organize, and coordinate multiple tasks. Excellent problem solving, time management, prioritization and organizational skills. Effective assessment skills with the ability to successfully establish goal planning with clients. Valid driver’s license.
