The case manager will determine the resources needed to meet the individualized needs of the participant. The case manager will take all necessary actions in assuring the implementation of the service plan including serving as an advocate for the participant to enable him/her to receive needed services; identification of program needs, service gaps and recommending strategies. A commitment to the RHD values should be demonstrated as job duties are performed. This is a hybrid position. Travel will be required for service observations and team meetings.\nEssential Duties and Functions\nTeam-Based Planning\nAct as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.\nAssist in the development of individual service plans and reviews in collaboration with the program participant, community service providers, and other stakeholders as determined in collaboration with program participant.\nDevelop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.\nEssential Duties and Functions:\nAct as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective.\nAssist in the development of individual service plans and reviews in collaboration with the program participant, community service provider and other stakeholders as determined in collaboration with program participant.\nDevelop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.\nFacilitate team process for the initial and annual development of the Individual Service Plan (ISP)\nOngoing monitoring of ISP and documenting results and intervene as necessary\nReport and review Critical Incident Reports (CIR) per protocol\nParticipate and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews as appropriate\nFocus on Personal Outcome Measures\nKnowledge and understanding of Charting the Life Course framework and Person Centered Thinking/Planning\nEvaluate the needs of those on caseload in order to remain successful in the community\nLink those on caseload to programs and supports that are needed and requested to meet their needs\nComplete required documentation of services, including input of data, reports, and plans into Therap\n\nQualifications\nBachelor’s degree and one year of experience OR equivalent combination of education and experience.\nIntermediate computer skills\nHave the ability to establish a HIPAA compliant home office\nValid driver’s license\n\nAbout Company:\nApis Services, Inc.
