As a member of our team, you'll play a key role in providing quality home healthcare, where patients are able to remain independent at their place of residence in the healthiest and happiest state possible. The Primary Care Registered Nurse (Primary Care RN) is the agency employee assigned primary responsibility for planning and implementing the patient's care. The Primary Care RN provides skilled nursing care to patients on an intermittent basis in their place of residence, in accordance with physician orders and the patient's plan of care, under the direction of the Supervisor of Clinical Services. The Primary Care RN also serves as the supervising nurse for homemaker-home health aides assigned to their patients. Benefits of Joining Our TeamFlexible schedulingCompetitive PayFree gym membershipTreated with respect and dignitySupported in the fieldOngoing training and development opportunities What We're Looking ForGraduate of an accredited school of professional nursingActive, unrestricted license to practice as a Registered Nurse in the State of ConnecticutMinimum of two (2) years of nursing experience, with at least one (1) year in home health care or a community health program that included care of the sick at homeCurrent BLS certificationValid Connecticut driver's license, reliable transportation, and current auto insuranceAbility to travel locally throughout the agency's service areaStrong clinical assessment, documentation, and communication skillsPassion for serving patients and families with compassion and professionalism What You'll Be DoingConduct initial home assessments and admit patients to serviceDevelop, implement, and revise individualized patient care plansProvide skilled nursing care requiring specialized clinical judgmentCoordinate care with patients, families, physicians, and the interdisciplinary teamEvaluate patient progress and adjust the plan of care as neededIdentify needs for additional services and make appropriate referralsDevelop written assignments for home health aides and supervise them in the home, including supervisory visits at least every four weeksPrepare timely, accurate clinical notes and progress documentationParticipate in case management conferences, in-service education, and the agency's quality assurance programEnsure compliance with all federal, state, and agency policies, including HIPAA and the Patient's Bill of Rights What You Can ExpectApplication Review: Our recruitment team will review your application within 1 business day of submission.Initial Phone Screening: If your application is shortlisted, you'll be contacted for a 15-20 minute phone screening within 2 business days.Interview: Qualified candidates will be invited to a virtual or in-person interview with the Supervisor of Clinical Services. This typically takes place within 3-4 business days of the phone screening.Final Decision & Orientation: We aim to make a final decision and begin orientation within 2 business days of the interview. All new employees receive a documented orientation covering agency structure, patient care policies, philosophy of care, personnel policies, and applicable state and federal regulations.Feedback: All candidates who reach the interview stage will receive feedback on their application status.
