As a Home Health Physical Therapist , you are a critical part of the care team - helping patients safely recover, regain mobility, and maintain independence where they feel most comfortable: at home. Deliver Patient-Centered Care Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential Develop and implement individualized care plans in collaboration with physicians and the care team Provide hands-on therapy to improve mobility, strength, balance, and independence Support safe transitions of care and contribute to discharge planning Partner closely with nurses, therapists, and physicians to ensure coordinated care Document patient care and progress accurately and timely within the EMR Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines Graduate of an accredited Physical Therapy program (DPT or Master's) Current PT license in the state of practice Minimum 1–2 years of experience (home health experience preferred) Valid driver's license, reliable transportation, and auto insurance You value autonomy and thrive in a field-based environment You bring both clinical excellence and compassionate care to every visit All employment decisions are made without regard to race, color, religion, sex, gender identity or expression, sexual orientation, national origin, age, disability, veteran status, or any other protected characteristic.
