This role ensures high-quality, compliant care delivery while supporting clients’ medical and daily living needs. The ideal candidate is a Licensed Practical Nurse with leadership experience in residential or community-based IDD settings and a flexible, hands-on approach.Key ResponsibilitiesClinical OversightConduct initial nursing assessments and ongoing reassessments for clientsReview, monitor, and ensure accuracy of Medication Administration Records (MARs)Manage medication reorders and coordinate with pharmaciesMonitor clients for changes in condition and report concerns appropriatelyAccompany residents to medical appointments and communicate follow-up care plansProvide clinical guidance to direct care staffStaff Supervision & TrainingSupervise field staff across residential homesTrain, onboard, and mentor new hiresConduct competency evaluations and skills validationsProvide coaching, corrective action, and performance feedbackServe as a clinical resource for staff questions and concernsOperations & CompliancePerform routine site visits to ensure compliance with state regulations and agency policiesEnsure homes are properly stocked with medical and household suppliesMonitor documentation, infection control, and safety practicesAssist with incident reports and quality assurance initiativesMaintain regulatory readiness for audits and surveysStaffing SupportProvide coverage for call-offs and staffing shortages when neededAssist with schedule coordination to maintain continuity of careQualificationsRequiredActive LPN license in good standingMinimum 2 years of nursing experience with IDD population and/or residential care settingsStrong knowledge of medication administration and documentationProven supervisory or leadership experienceReliable transportation and ability to travel between multiple locationsFlexible availability, including evenings and occasional weekendsPreferredExperience in group homes, waiver programs, or long-term residential settingsKnowledge of Medicaid waiver servicesStrong organizational, leadership, and communication skillsAbility to manage multiple priorities independentlyPhysical & Scheduling RequirementsAbility to lift up to 50 lbs as neededStanding, walking, bending, and driving between locationsOn-call availability as requiredMust be adaptable to fluctuating schedules and staffing needsWhat We OfferCompetitive compensation based on experienceCompany vehicle Supportive management teamPurpose-driven work serving the IDD communityFlexible work from home options available.
