DescriptionJob SummaryThe Medical Records Clerk is responsible for maintaining accurate, secure, and up-to-date patient medical records in compliance with clinic policies and healthcare regulations. This role supports clinical staff by ensuring timely access to patient information and protecting patient confidentiality.Key ResponsibilitiesCreate, update, and maintain patient medical records (electronic and/or paper)Ensure accuracy, completeness, and proper organization of medical chartsScan, index, and upload documents into the Electronic Health Record (EHR) systemRelease medical information in compliance with HIPAA and clinic policiesRespond to record requests from providers, patients, insurance companies, and legal entitiesTrack and audit records for compliance and quality controlCorrect documentation errors and follow up on missing informationAlways maintain confidentiality and data securityAssist with record retention, archiving, and destruction proceduresCollaborate with clinical and administrative staff as neededRequirementsQualificationsRequiredHigh school diploma or GEDBasic knowledge of medical terminologyProficiency with computers and EHR systemsStrong attention to detail and organizational skillsAbility to maintain confidentiality and comply with HIPAA regulationsEffective communication and customer service skillsPreferred1+ year experience in a medical office, clinic, or healthcare settingFamiliarity with insurance forms, referrals, and clinical documentationExperience with EHR systems (e.g., Cerner)
