We are building a durable care platform designed to perform in constrained healthcare environments and to lead the shift toward accountable, value-based care.Role Description The Senior Director of CalAIM Programs will join Zócalo Health during a period of rapid growth and increasing operational complexity. This role exists to ensure consistent execution of ECM, Community Supports (CS), and CHW programs across geographies and health plans.As part of a fast-growing team, you will be responsible for leading the execution layer of care delivery, ensuring workflows are created, updated, and followed, teams are accountable, and operational issues are surfaced and resolved quickly.This position reports to the Chief Operating Officer.The Senior Director, CalAim Programs will contribute in the following ways:Execution & Operational PerformanceOwn day-to-day execution of ECM, CS, and CHW programs across all geographiesEnsure visits, engagement, and care delivery activities are completed consistentlyMonitor operational KPIs (visit completion, time to first visit, engagement rates)Identify and resolve breakdowns in execution across teamsTeam Leadership & AccountabilityDirectly manage CHW Team Managers and Senior Manager of Community SupportsSet clear expectations for performance, productivity, and team managementHold managers accountable for outcomes, coaching, and corrective actionEnsure consistent management practices across geographiesWorkflow Adherence & Operational ConsistencyEnsure workflows defined by Payor Operations are executed consistently across all teamsIdentify where workflows are not being followed and drive correctionProvide structured feedback to Payor Operations on workflow gaps or execution challengesPartner on refinement of workflows without owning designCross-Functional PrioritizationMake tradeoff decisions across execution, capacity constraints, and workflow requirementsEscalate issues to Payor Operations, CHW Workforce, or Clinical Operations based on root causeCoordinate with other functions to ensure alignment without overlapping ownershipIssue Resolution & EscalationIdentify system-level issues impacting care delivery (staffing gaps, workflow gaps, quality issues)Assign clear ownership based on root cause and track resolutionEscalate unresolved issues to COO or cross-functional leadership when neededQualificationsDirect experience operating CalAIM programs (ECM and/or Community Supports) in California10–12+ years of experience in healthcare operations with a focus on care delivery or population health5+ years managing managers or multi-layered teams in a distributed or field-based environmentDemonstrated ownership of end-to-end program execution, including performance management and operational outcomesExperience driving consistent execution across multiple geographies or regionsStrong familiarity with Medicaid populations, documentation requirements, and care coordination workflowsProven ability to manage high variability environments (multiple plans, changing requirements, operational complexity)Track record of making real-time operational decisions under competing constraints Preferred QualificationsExperience operating within a multi-plan environment with differing requirementsExperience in value-based care or risk-bearing modelsExperience scaling operations in a high-growth or early-stage organizationWhat you can expect from Zócalo HealthEquity compensation packageComprehensive benefits including medical, dental, and vision 401kFlexible PTO policy - take the time you need to recharge$1,000 home office stipendWe provide the equipment needed for this role.Opportunity for rapid career progression with plenty of room for personal growth.You must be authorized to work in the United States.
