Company:Providence at Home with CompassusThis is a collective bargaining positionDay Shift5 days/flexibleFull TimePosition SummaryThe Admissions RN is responsible for coordinating and processing hospice referrals to ensure timely and seamless patient admissions. This role serves as a key clinical resource for referral intake, reviewing and accepting referrals from various sources via phone, fax, and electronic systems. The Admissions RN collaborates closely with referral sources, physicians, intake staff, and the clinical team to coordinate initial hospice assessments and ensure compliance with hospice regulations and organizational standards.Key ResponsibilitiesReceive, review, and prioritize hospice referrals to support timely admissions and continuity of careRespond promptly and professionally to referral inquiries and intake callsEnter and maintain accurate patient demographic, clinical, and insurance information within the EMRObtain physician orders, document Certification of Terminal Illness (CTI) information, and ensure referral documentation is complete and compliantCoordinate admission scheduling and communication with the clinical team, Patient Care Supervisors, and Patient Care CoordinatorsReview and co-sign referrals entered by LVNs and non-clinical intake staff as requiredCollaborate with billing, insurance verification, pharmacy, and PBM partners to facilitate smooth patient onboardingApply Medicare, state, and hospice regulatory guidelines when reviewing and processing referralsMaintain accurate, timely documentation and support efficient workflow within the intake and admissions processPerform additional duties as assignedQualificationsActive Registered Nurse (RN) license in applicable state requiredMinimum 1 year of RN experience requiredHospice, home health, or admissions/intake experience preferredStrong clinical judgment, organizational, and communication skillsExperience working within EMR systems and managing multiple priorities in a fast-paced environmentBLS/CPR certification required within 30 days of hirePhysical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.
