Access to Care
The Medical Outreach Case Manager (MOCM) is a professional social services role responsible for providing case management services and achieving positive outcomes for assigned clients. This position involves not only managing patient caseloads but also addressing complex medical and social challenges, removing barriers to care, and meeting patients' health needs. Through these efforts, the Access to Care program strives to ensure access to comprehensive, affordable healthcare, ultimately enhancing the quality of life in our communities.
What you will do:
Identify appropriate interventions within program/department parameters, protocols, and procedures. Interventions may include making/tracking referrals to hospital and community resources, agencies and programs, providing health education, and assistance in completion and follow through of in-depth social service applications
Contribute to the partnership among care providers, clients, and families to provide safe and effective care; may participate as a member of an interdisciplinary team to support clients
Responsibilities:
Develop and maintain good working relationships with health care providers, community based agencies, and other departments/disciplines
Provide timely and appropriate client assessments, care planning, and progress notes as part of care coordination
Comply with Employee Health, Human Resources, and HIPAA polices and protocols at all times, and involve MMC Management when responding to complex client situations
Required Minimum Knowledge, Skills, and Abilities (KSAs) – “What you will need”:
Education: Bachelor’s degree required, with a specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing or closely related field preferred. In lieu of a Bachelor’s degree, 3 – 5 years directly relevant experience in a healthcare setting, with 6 months case management experience may be considered. (see Experience below)
License/Certifications: Eligibility for LSW licensure preferred. Required to travel off-site and be comfortable in unconventional settings or situations. Valid Driver’s License and reliable vehicle required.
Experience: With a Bachelors degree: 2 years experience in a community health setting, to include 1 year of case management experience.
In lieu of Bachelors degree: 3 -5 years directly relevant experience in a healthcare setting, to include 6 months of case management experience. Relevant experience provides advanced knowledge of community outreach resources, federal programs and skills in assisting clients to make informed decisions about their health and health care.
