Provide clerical, administrative, member and team support for the Diabetes Prevention Program. The goal of the program is to prevent or delay type 2 diabetes by supporting members. Interacts with internal departments to accomplish program goals and track member progress. Helps coordinate member enrollment, quarterly check ins, and other data documentation as required by the CDC. This is a FT WFH role.\n\nPay Range\n$23.34- $26.26 hourly (depending on experience)\nActual pay is based on qualifications. Applicants who do not exceed the minimum qualifications will only be eligible for the low end of the pay range.\n\nPlease fill out an application on our company page, linked below, to be considered for this position.\n\nhttps://j.brt.mv/jb.do?reqGK=27783990&refresh=true\n\nBenefits:\n\nMedical, Dental, Vision, Pharmacy, Life, & Disability\n401K- Matching\nFSA\nEmployee Assistance Program\nPTO and Company Paid Holidays\n\nRequired Skills, Experience & Education:\n\nBachelor's degree in health promotion preferred or equivalent work experience in the health care industry.\nExperience in program support, ability to track & coordinate multiple projects with extended timelines, and healthcare operations preferred.\nAbility to work collaboratively and independently.\nExcellent communication, interpersonal, and presentation skills.\nStrong cultural competence and understanding of the importance of using Diversity, Equity, and Inclusion (DEI) concepts to guide and inform work.\nThorough knowledge and understanding of health behavior change theory.\nExperience with Microsoft Office products.\nAbility to learn proprietary databases and maintain confidentiality.\nPreferred Requirements:\nExperience in health coaching or field related to health coaching.\nExperience in health insurance.\nSpeaker of both English and Spanish.\nMaintains a professional appearance and demeanor in all internal or external interactions.\n\nPrimary Functions:\n\nSupports the coordination and implementation of Diabetes Prevention Program.\nConducts outreach and new member enrollment with Pre-D program eligible members via phone, email, mail and virtually to support member recruitment, enrollment, retention and completion of the Pre-D program.\nAssists care team with general clerical support and program operations.\nRoutinely documents member progress, records and verifies the required member data to submit externally (CDC, employer groups) and report internally to various committees, (Medical Quality Improvement Committee, Quality Steering Council, etc.), Supervisor and other leaders, as needed.\nIn collaboration with the Supervisor and data team, ensures all data is submitted accurately and timely.\nDevelops and monitors various key performance indicators including outreach, enrollment, attendance, retention, completion, disenrollment and health outcomes.\nManages program calendar, emails, and telephone lines.\nComplies with all applicable laws and regulations, including those governing participant privacy and data security.\nAbility to work a flexible schedule including early mornings, over the lunch hour and early evenings to meet the needs of program participants.\nPerforms other job-related duties, as assigned.\n\nWorking Conditions & Contact with Others\n\nOffice environment with extensive close PC and keyboard work, constant sitting, and phone work. Must be able to navigate multiple screens. Work in excess of standard work week in order to meet business needs.\nInternally with all departments and subsidiary companies.
