This position oversees the Program & Data Coordinators and works closely with the Executive Director, CHW supervisors, and partner agencies to keep our work aligned with CHAP’s mission and the Pathways Community HUB model.\nEssential Responsibilities\nProgram Oversight\nOversee day-to-day operations for CHAP’s OCMH and Healthy Families America (HFA) programs.\nMonitor program deliverables, timelines, and quality standards to ensure we stay on track with grant expectations.\nSupport Coordinators in problem-solving barriers and improving workflows.\nCollaborate with supervisors and CHWs to strengthen enrollment, documentation, and participant engagement.\nData & Reporting\nEnsure accurate, timely data entry and document management across all CHAP programs.\nReview data for trends, quality issues, and compliance risks.\nPrepare monthly, quarterly, and annual reports for OCMH, HFA, PCHI, and other funders as needed.\nMaintain strong working knowledge of CCS and other data platforms used by CHAP.\nTeam Leadership\nDirectly supervise Program & Data Coordinators, including regular check-ins, performance development, and coaching.\nProvide guidance on program expectations, documentation needs, and workflow changes.\nSupport training needs and help onboard new staff connected to program operations.\nLead with a supportive, solutions-focused approach consistent with a Just Culture environment.\nQuality & Compliance\nPartner with the Executive Director and Operations & Training Manager to support quality improvement efforts.\nHelp maintain policies, procedures, and program manuals related to OCMH, HFA, and broader HUB functions.\nEnsure CHAP meets requirements tied to fidelity, confidentiality, and data security.\nCollaboration & Partnerships\nSupport communication with funders, evaluators, community partners, and referral sources.\nParticipate in meetings, site visits, and audits for OCMH, HFA, and other programs.\nStrengthen CHAP’s relationships with healthcare and community partners through consistent follow-up and professional communication.\nWork Environment\nThis position operates in a professional office setting and frequently interacts with staff, Care Coordination Agencies (CCAs), and community partners. The Program & Data Manager uses standard office equipment, computers, CCS, reporting dashboards, and communication tools to perform daily duties The role requires regular onsite work with occasional local travel for meetings, training sessions, and agency support. The work involves periods of focused data review, virtual and in-person collaboration, and participation in team meetings and quality improvement activities.\nPhysical Demands\nTravel within the service region for meetings, presentations, and events.\nOccasional evening or weekend work may be required.\nAbility to communicate effectively, move throughout office and community settings, and lift up to 10 pounds as needed.\nReasonable accommodations may be made to enable individuals with disabilities to perform essential functions.\nEducation & Experience\n· Bachelor’s degree in public health, social work, human services, or a related field (or equivalent experience).\n· Experience in program management, data systems, or grant compliance preferred.\n· Strong understanding of community health workers and the Pathways Community HUB model is a plus.\n· Excellent organizational skills and comfort managing multiple priorities at once.\n· Strong communication skills and the ability to lead, coach, and support staff.\nOther Duties\nThis position may include additional responsibilities not listed here, based on organizational needs.\nEmployment At-Will\nThis job description does not constitute an employment contract.
