Benefits for Full-Time Staff 401(k) with company match Health, Dental, and Vision insurance Life insurance Paid time off Flexible scheduling support Fleet Vehicle program for qualifying staff Key Responsibilities Conduct comprehensive psychosocial assessments and develop individualized care plans, including interventions and patient-centered goals Provide emotional support, counseling, and resource coordination for patients and families Work both independently and collaboratively within an interdisciplinary team Utilize community resources and professional experience to proactively address patient needs Maintain accurate and timely documentation Qualifications Minimum of a Bachelor’s degree in Social Work (BSW); a Master’s degree in Social Work (MSW) is preferred Minimum of 1 year of social work experience – Required Home health or healthcare experience – Preferred, but not required Valid driver’s license, proof of auto insurance, and reliable transportation – Required Strong commitment to improving patient quality of life Ability to work independently while thriving in a team environment Detail-oriented approach to assessments and care planning Proactive mindset grounded in knowledge, experience, and available resources Salary - $50,000-$75,000 Pay: $50,000.00 - $70,000.00 per year License/Certification: MSW (Preferred) BSW (Required) Location: Pittsburgh, PA (Required) Work Location: On the road
