Clinical Care Assistant

Job Description Description:The Clinical Care Assistant provides direct patient care in the home setting for individuals recently discharged from skilled nursing facilities. This role supports continuity of care by delivering wound care, care management support, and other approved medical services under the direction of a licensed Medical Director. The Clinical Care Assistant works independently in patient homes and plays a vital role in ensuring patient safety, adherence to care plans, and timely communication with the broader care team.This position is ideal for a Clinical Care Assistant who is confident in practicing independently, comfortable traveling within a defined territory, and passionate about delivering high-quality, patient-centered care in the home environment.Provide in-home clinical services to patients following discharge from skilled nursing facilitiesPerform wound care, dressing changes, and monitor healing in accordance with established care plans and protocolsConduct patient assessments including vital signs, symptom monitoring, and medication reconciliationSupport ongoing care management by identifying changes in patient condition and escalating concerns appropriatelyEducate patients and caregivers on wound care, infection prevention, medication adherence, and follow-up careAccurately document all patient encounters, observations, and services provided in the medical recordCoordinate with providers, nurses, and care coordination teams to ensure continuity and quality of careMaintain compliance with infection control, safety standards, and organizational policiesTravel independently to patient homes within assigned service areaRequirements:Current Medical Assistant certification required (CMA, RMA, or equivalent)Minimum 1–2 years of clinical experience as a Clinical Care Assistant requiredDemonstrated experience with in-home and hands-on patient careComfortable working independently and entering patient homes aloneStrong clinical judgment, attention to detail, and ability to follow care protocolsExcellent communication and patient education skillsBasic computer skills and comfort with electronic documentationTravel & Work EnvironmentFull-time, field-based positionMust be willing and able to travel up to 1.5 hours driving distance from home within assigned territoryReliable transportation and valid driver's license requiredKey Attributes for SuccessDependable, self-directed, and highly organizedCompassionate and professional bedside mannerAdaptable to changing patient needs and home environmentsStrong commitment to patient safety and quality careScope of Practice NoticeAll services are provided in accordance with Clinical Care Assistant scope of practice and under appropriate clinical oversight.
