Nurse Practitioner
Our exclusive partner is looking to add a Nurse Practitioner to their growing team. They are the first health center in IL to offer a PACE program for individuals over the age of 55. This program supports the geriatric population who need nursing home-level care but wish to remain living at home.
Position Overview: We're seeking a dedicated Nurse Practitioner to join our partner's PACE team. In this role, you will deliver primary medical care to older adults, emphasizing prevention, wellness, and a holistic approach to health. You'll be a key member of our Interdisciplinary Team, collaborating with other healthcare professionals to create personalized care plans and improve the quality of life for our participants.
Key Responsibilities:
Provide comprehensive primary care, including physical exams, diagnoses, and treatments.
Emphasize preventative care and patient education.
Collaborate with our Interdisciplinary Team to develop individualized care plans.
Participate in quality improvement initiatives and team meetings.
Engage in community activities and continuous professional development.
Requirements:
Bilingual (English and Spanish) required.
Certified in CPR/BLS by the American Heart Association.
Valid driver’s license and current automobile insurance.
Graduate degree or post-master certificate from an accredited Nurse Practitioner or Physician Assistant program.
At least 2 years of experience working with older adults.
