Residents are employed in the local community, and you will help them maintain their housing, navigate challenges, and move toward greater independence. Abilities\r\nMinimum of three (3) years' experience working with under-resourced or low-income populations; experience working with individuals experiencing homelessness preferred.\r\nExperience in residential and/or housing programs.\r\nKnowledge of Housing First and harm-reduction principles.\r\nWorking knowledge of SSI, SSDI, and SDI.\r\nExperience with data entry systems (HMIS, Salesforce, housing-focused case management in one-on-one settings for up to 24 single adults and group settings.\r\nIncorporate CalAIM care coordination and housing stabilization strategies into individualized housing and care plans, in collaboration with healthcare, behavioral health, and community-based partners.\r\nIdentify and address barriers to housing stabilization, including chronic homelessness, mental and physical health needs, substance use disorders, under-employment, lack of income, and other systemic barriers.\r\nSkillfully prevent and/or resolve conflicts using a strengths-based approach.\r\nManage a full caseload and bi-weekly/monthly meetings, with strong organization, time-management, and prioritization skills.\r\nParticipate in weekly supervision, program meetings, all-staff meetings, and required training.\r\nRespond to urgent and crisis situations, including coordination with emergency medical teams, fire department, law enforcement, psychiatric services, and community partners as needed.\r\nPrevent and resolve conflicts using a strengths-based, trauma-informed approach.\r\nProvide supportive services, guidance, and referrals to community resources.\r\nEngage program participants regularly and consistently to support program participation and progress toward housing stability.\r\nAble to maintain professional boundaries and set personal goals.\r\nMaintain accurate and timely data entry in HIPPO (Salesforce database) and HMIS.\r\nMeet all documentation and case management deadlines, including intake, enrollment, reassessments, housing goals, and discharge documentation.\r\nParticipate in quality assurance, monitoring, and audit processes, including responding to documentation reviews and corrective actions.\r\nCase Management, Housing-Focused Services, and Outcomes\r\nDevelop, implement, and regularly update individualized, client-centered housing and care plans.\r\nMonitor, document, and adjust plans proactively to address changing needs or barriers.\r\nCoordinate services with internal teams and external partners to ensure continuity of care and timely referrals.\r\nDocument housing and service outcomes to support program performance measurement and funder reporting.\r\nDevelop, implement, and monitor financial literacy services tailored to covering topics such as budgeting, credit repair, savings strategies, and employment preparedness.\r\nAssist program participants in housing retention, job readiness skills, job placement and retention services, education on living skills, navigating resources and referrals, eviction diversion, and addressing physical and mental health issues.\r\nProperty Management and Housing Placement\r\nAssist the Senior Director of Housing with client identification and screening for housing eligibility, leasing, and client move-ins.\r\nAssist the Senior Director with refining program policies and practices, unit setup and purchasing, and facility troubleshooting.\r\nPromptly submit documentation to the Marin Housing Authority for the issuance of client vouchers and rental calculations/recalculations.\r\nAssist with property management, including but not limited to creating and tracking maintenance requests, maintaining site presence across the housing campus, tracking client rental payments, issuing legal notices, and documenting tenancy challenges.\r\nProactively monitor building needs, rounds, coordinating with facilities.\r\nAbility to read and interpret documents, write routine reports and correspondence, and speak effectively before small groups of clients or employees of the organization.\r\nCommunicate clearly and professionally with supervisors, team members, volunteers, and program
