The Area Agency on Aging wants to talk with you!\r\nThis position is established to support case managers by implementing authorized changes, services and contacts for PASSPORT and Assisted Living Waiver individuals.\r\nDetailsConducts phone calls with individuals following hospital/nursing home discharge to complete care transition activity.\r\nMails zone management tool to individuals following discharge with qualifying diagnosis.\r\nAssists the case manager by providing individual/informal support information to assist in gaining access to any necessary services regardless of funding source which enables individuals to remain in their community living arrangement.\r\nAccurately and timely completes changes to the amount, scope, and duration of waiver services at the direction of the case manager.\r\nDocuments emails or voice mails from service providers in case notes and make corresponding adjustments to the service plan.\r\nContacts service providers or individuals/informal support to monitor service delivery and resolve service provision problems.\r\nCompletes service suspends in collaboration with case manager.\r\nConducts timely home visits, Level of Care and Comprehensive assessments as needed, according to program specifications.\r\nAccurately maintains all appropriate documentation in formats and time frames prescribed by the PAA and the Ohio Dept. of Aging.\r\nUtilizes electronic tools and software to complete tasks.
