Profe ssiona l Community Services (QMPH-CS)Qualifications, skills, and all relevant experience needed for this role can be found in the full description below.The QMHP-CS is primarily responsible for providing family oriented, individual and/or group rehabilitativemental health services, skills training and development, medication training, parenting skills training, andintensive case management services to clients of the agency per state specified standards andrequirements. It is the responsibility of this position to provide clinically appropriate services, a minimumof two times per month, to all consumers assigned for services.The qualified candidate has a Bachelor’s degree in rehab counseling, psychology, social work, or relatedfield. The ideal candidate will have experience working in community mental health, hold a master’sdegree, and be provisionally licensed as an LPC- Intern. Intern supervision will be provided as a benefit ofemployment. General understanding of adult and child mental health disorders, treatment of disorders,and documentation of treatment provided. Knowledge of confidentiality and ethical guidelines for practice.Should possess basic therapeutic skills, verbal and written communication skills, and organizational skills.Trained in working with individuals with Severe and Persistent Mental Illness, Children with SevereEmotional Disturbances, and delivery of services based on curriculum.RESPONSIBILITIES• Provide and exhibit Engagement skills as identified by the Wraparound Care Coordinator Tool Kit• Provide and exhibit Implementation skills as identified by the Wrapround Care Coordinator TookKit• Reports suspected child abuse or neglect immediately to the Abuse and Neglect Hotline andDirector• Ensure compliance with all rules and regulations of DSHS including licensure rules andregulations and suspected fraudulent practices• Responds to on-call cell phone immediately• Attend all Team, Individual, and Training Meetings as required and scheduled• Provide and exhibit Planning skills as identified by the Wraparound Care Coordinator Tool Kit• Document services provided with the understanding that all clinical notations are considered legaldocuments and can be used in a court of law. All documentation should be true and accurate andunder no circumstance falsified in any manner• Provide and exhibit Transition skills as identified by the Wraparound Care Coordinator Tool Kit• Maintain client confidentiality at all times and be familiar with all HIPPA laws and regulations• Maintain professional certification and/or licensure, as this applies• Prepare and present all assigned cases for review by a supervisor, as required on a prescribedbasis• Provide targeted (intensive) Case Management services to families• Opens and processes mail / e-mail daily in a timely manner• Participate in Child and Family Team meetings as appropriate• Provide transportation as required• Maintains compliance with all United Human Services policies procedures, and regulatoryrequirements including all state and federal laws• Maintains confidentiality as required by government law andCompany Description Offer will differentiate Commensurate with experience. xaygatp Company Description Offer will differentiate Commensurate with experience.
