The ideal candidate will possess a strong background in senior home care settings, with experience in managing complex client cases, responsible to assign clients and their scheduled services. The position helps with reassessing and updating care needs and must able to know how to handle senior care. As a Case Coordinator, you will play a vital role in facilitating seamless patient care transitions, coordinating services and scheduling, and ensuring compliance with company regulations.
