They maintain and support a caseload of clients enrolled in Medicaid programs and conduct regular check-ins to update care plans as needs change.\nIf you enjoy a role that gets you out of the office, this position offers frequent, planned travel to client locations within a three hour radius. You’ll be supported with either a company vehicle or mileage reimbursement to make travel smooth and manageable.\nDuring your first 12 weeks of hire, you’ll complete new hire training that includes up to four in-person sessions in Dallas.\nJOB DUTIES\nMaintain and coordinate a caseload of clients enrolled in Medicaid programs\nTravel frequently to client homes and community locations (approximately 90% of the role)\nParticipate in a 12-week new hire training program, including occasional travel to Dallas\nCoordinate clients’ in-home care needs in alignment with Federal/State regulations, MCO program rules, and contract requirements\nAssist with daily intake and referral processes to support positive business growth\nPromote a collaborative and supportive office environment\nSupport day-to-day operations and client advocacy\nWork independently and meet position requirements with minimal oversight\nCollaborate with stakeholders to support service delivery\nCommunicate effectively—both written and verbal\nComply with all applicable legal requirements, standards, policies, and procedures\nDemonstrate dependability and timely follow-through\nUse sound problem-solving and decision-making skills\nExhibit strong computer proficiency\nManage complex service programs and special projects\nMarket services to referral sources as needed\nMaintain necessary knowledge to coordinate workflow\nParticipate in professional development and training activities\nPrioritize and multitask effectively in a fast-paced environment\nProvide excellent customer service to internal and external stakeholders\nRepresent the organization at stakeholder meetings, health fairs, and provider fairs\nEnroll clients and employees in services, web portals, and EVV systems as needed\nConduct client assessments and home visits as required\nAnswer and make calls, including occasional video calls\nOther duties as assigned\nQUALIFICATIONS\nDemonstrate 1+ years of experience of in-person customer/client support\n1+ year of experience supporting individuals with disabilities or the elderly\nDemonstrated commitment in a team environment by showing initiative, collaborating effectively, and contributing to a supportive and productive workplace\nEquivalent combinations of education and experience considered\nBilingual candidates preferred in certain locations\n\nThe incumbent typically works in an office environment and uses a computer, telephone and other office equipment as needed to perform duties. The noise level in the work environment is typical of that of an office. Incumbent may encounter frequent interruptions throughout the workday.
