Working collaboratively with participants, families, providers, and community partners, the Case Manager assists individuals in navigating behavioral health, housing, employment, healthcare, transportation, and social service systems. The Case Manager promotes person-centered care and provides coordination, support, and advocacy to help participants achieve their goals and improve overall quality of life.\n\nRESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS\nDirect Duties\nConducts comprehensive assessments to identify participant strengths, needs, preferences, and barriers to success.\nDevelops, implements, and monitors individualized service and treatment plans.\nAssists participants in establishing recovery goals and achieving positive outcomes.\nAdvocates for participants to ensure access to appropriate services, supports, and community resources.\nProvides community-based case management and support services.\nConducts screenings, referrals, and service coordination activities.\nConnects participants to behavioral health, medical, housing, employment, transportation, educational, and social service resources.\nAssists participants with obtaining benefits and entitlements, including but not limited to Medicaid, Social Security benefits, housing assistance, and community resources.\nCoordinates services among providers, families, caregivers, and support systems.\nMonitors participant progress and adjusts service plans when necessary.\nMaintains regular participant contact through office, community, home, and virtual visits as appropriate.\nSupports crisis intervention and resource coordination when needed.\nProvides education regarding available services, resources, and recovery supports.\nAccurately documents services provided, participant progress, and service outcomes.\nMaintains compliance with agency, licensing, accreditation, and funding source requirements.\nDocuments time worked accurately.\nOther duties as assigned by management.\nOther Responsibilities\nParticipates in multidisciplinary treatment team meetings and care coordination activities.\nCollaborates with internal programs and external community providers to ensure continuity of care.\nEstablishes and maintains relationships with community service agencies and referral sources.\nIdentifies service gaps and participates in program development and quality improvement initiatives.\nCompletes assessments, case notes, treatment plans, discharge planning, and other required documentation.\nAssists with program reporting and compliance activities.\nMaintains confidentiality and protects participant information in accordance with HIPAA and agency policies.\nDemonstrates professionalism, ethical conduct, and sound judgment.\nParticipates in required trainings, supervision, and professional development activities.\nSupports overall program operations and administrative responsibilities.\nOther duties as needed to support participant success and program effectiveness.\nADHERE TO AND FOLLOW\nThe By-Laws of the Corporation and as set forth in the Articles of Incorporation.\nJob Qualifications\nEducation and Experience\nBachelor's Degree in Human Services, Social Work, Psychology, Counseling, Sociology, Behavioral Health, or a related field required.\nOne (1) year of case management experience preferred.\nMinimum two (2) years of experience supporting individuals in behavioral health, addiction recovery, mental health, or related human service settings preferred.\nValid driver's license with acceptable driving record required.\nAbility to work effectively in both
