Position TitlePrimary Care Physician – Multiple PositionsPosition OverviewProvide primary care services to senior/geriatric patients, including preventive and therapeutic medical services. Assess patients for organic or congenital disorders, determine the nature and extent of injury or disease, and conduct examinations. Refer patients to diagnostic imaging, laboratory tests, and medical specialists when appropriate. Prescribe medications and recommend dietary and activity programs based on clinical findings.ResponsibilitiesConduct comprehensive examinations of senior patients.Diagnose and treat age‑related diseases and disabilities.Prescribe medications and recommend lifestyle changes.Coordinate referrals to specialists and coordinate follow‑up care.Maintain accurate medical records.RequirementsMedical Degree or foreign equivalent.Board Certified or Board Eligible in Family Medicine or Internal Medicine.Illinois State Medical license and DEA license.Location2734 W 87th Street, Chicago, IL 60652.
