This position works directly with patients and on behalf of patients to reinforce treatment goals and access resources in the community, promoting a recovery-oriented lifestyle.\n\nESSENTIAL RESPONSIBILITIES AND TASKS:\n\nAssist patients in accessing alcohol and drug free housing, medical and emergency services and other community resources, such as sourcing food, submitting rental and employment applications, and utilities assistance.\nAssist patients meeting recovery goals.\nConsistent attendance and punctuality are required of this position.\nFacilitate connection to the recovery community and other supports that may reinforce the goals of treatment.\nTeach and model positive recovery behaviors while maintaining appropriate professional boundaries.\nFacilitate peer recovery and life skills groups.\nBuild and maintain positive working relationships with individuals and organizations in the recovery community.\nParticipate in team meetings, case conferences, work groups, committees, trainings, and applicable community organized events.\nUse agency network and electronic health records to document all relevant contacts with or in regard to patients.\nPromote community integration and positive outcomes in the areas housing, employment, education, family and social support and recovery.\nAssist in the tracking of critical data elements necessary for program reporting.\nAccompany and drive patients to recovery related appointments.\nAchieve outcomes using methods that align with CODA’s mission and values:\nCommunicate accurately and positively about recovery and the use of evidence-based treatments.\nUse data and science to identify barriers, overcome obstacles, and inform decisions.\nBe financially responsible; avoid waste, document accurately, seek new opportunities to serve.\nDemonstrate compassion and respect for others.\n\nSECONDARY RESPONSIBILITIES AND TASKS:\n\nAssist the team by providing short-term relief coverage and/or basic training for other staff members.\n\nREQUIRED EDUCATION AND EXPERIENCE:\n\nHigh school diploma or GED.\n\nCOMPETENCIES AND SKILLS:\n\nAdhere to regulatory requirements and maintain professional, ethical and behavioral standards of conduct.\nAbility to be attentive to and identify high risk situations, communicating with the appropriate channels in a timely manner.\nAbility to intervene in such a way that demonstrates sensitivity and respect while keeping a clear line of communication between supervisor and treatment team.\nAbility to demonstrate a non-judgmental and respectful attitude with patients at all times.\nAbility to establish rapport quickly, maintain appropriate boundaries, and remain nonjudgmental to patients.\nAbility to utilize both oral and written communications skills to effectively communicate with patients, co-workers, community resources, supervisors, and various other individuals encountered in the course of performing work while maintaining patient confidentiality and establishing professional rapport.\nAbility to maintain professional communications and boundaries while managing complex situations and interacting with individuals diagnosed with substance use or co-occurring conditions.\nAbility to operate a computer, utilize basic MS Office programs, and effectively navigate and maintain patient records in an electronic medical records system.\n\nREQUIRED CERTIFICATIONS AND LICENSES:\n\nAttain CRM or PSS certification within 30 days of hire (if not already certified).\nMay be required to attain a Basic Life Support (BLS) certification or ability to acquire within 90 days of hire.\nCrisis Prevention training completion within 90 days of hire.\nA valid Oregon or Washington driver’s license (Withdrawal Management Program may or may not require).\n\nPHYSICAL DEMANDS:\n\nMust be able to safely operate a motor vehicle, including a passenger van for residential settings.
