The organization fosters transformative changes at both individual and community levels, contributing to broader community advancement.Role DescriptionThis is a full-time, on-site role located in New Haven, CT, for a Family Case Manager. The Family Case Manager will be responsible for conducting comprehensive case management services, ensuring support to families and individuals in need of assistance. Daily responsibilities will include providing guidance in family services, coordinating resources, and addressing critical social work matters. The role involves creating personalized action plans, collaborating with community partners, and maintaining detailed documentation of client progress.QualificationsStrong foundation in Social Work and Family Services to ensure effective and empathetic client supportExperience or knowledge in community service and resources, and working with vulnerable family structuresProven skills in Case Management and delivery of Case Management ServicesExcellent problem-solving, organizational, and communication skillsAbility to prioritize and meet deadlines in a community-centered environmentBachelor’s degree in Social Work, Human Services, or a related field (preference)Fluency in Spanish is highly desirablePrevious experience working with diverse and underserved communities is an asset
