This role requires excellent clinical skills, the ability to work collaboratively with other healthcare professionals, and a passion for supporting patients and their families.\nDuties\nAs a Licensed Practical Nurse or Registered Nurse, your responsibilities will include:\nProviding direct patient care under the supervision of registered nurses and physicians.\nAdministering medications and treatments as prescribed, including IV infusion and injections.\nConducting patient assessments, monitoring vital signs, and documenting medical records accurately.\nAssisting with daily living activities for patients in long-term care, assisted living, or home care settings.\nCollaborating with interdisciplinary teams to develop and implement individualized care plans.\nSupporting patients with developmental disabilities, Alzheimer’s care, dementia care, and memory care.\nPerforming procedures such as catheterization, tube feeding, phlebotomy, and specimen collection.\nEnsuring compliance with HIPAA regulations and maintaining patient confidentiality.\nParticipating in discharge planning and providing health coaching to patients and families.\nRequirements\nTo be considered for this position, candidates must meet the following qualifications:\nValid LPN license in the state of practice.\nExperience in critical care, acute care, or long-term care settings is highly desirable.\nFamiliarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.\nKnowledge of medical terminology and ICD coding (ICD-9/ICD-10).\nStrong skills in patient assessment and basic life support (BLS).\nExperience working in various specialties including pediatrics, geriatrics, emergency medicine, or hospice care is a plus.\nExcellent communication skills and the ability to work effectively within a team environment.\nA commitment to ongoing professional development and adherence to best practices in nursing.
