{"schemaVersion":"jobsearcher.job.v1","id":"7cc247fff894e67fde8cb956","url":"https://jobsearcher.com/jobs/7cc247fff894e67fde8cb956","canonicalUrl":"https://jobsearcher.com/jobs/7cc247fff894e67fde8cb956","title":"Program Manager (QIDP)","description":"Program Manager\nReports to: Regional Director Classification: Full-time; non-exempt\nSetting: Remote (in community and at home office)\nSummary: A Program Manager works as a quality assurance liaison with Individuals, families, guardians, advocates, case managers, Direct Support Professionals (DSP), other service providers and community resources to create a cohesive support team for the success of theIndividual.\nKey Responsibilities:\nIndividual & DSP Support\nBuild relationships with Individuals and understand their interests and needs; carry a caseload of at least 35 Individuals\nAssure that DSPs adhere to waiver rules and regulations; be a resource for DSPs as LEL employees and as support staff\nConnect with every Individual/family every month and visit at least quarterly\nCoordinate with IST/Individual Support Team (family members, case managers, behavior support specialists and other stakeholders)\nAssist Individuals and their family members with maximizing their waiver funds\nProvide ideas and guidance to help Individuals achieve the Good Life\nOccasionally provide direct care and support if no DSP or natural supports are immediately available\nRecordkeeping\nEnsure \"Blue Books\" with critical information about each Individual is at their home and up to date\nCollaboratively establish Individuals' goals\nEnsure drills and assessments are completed monthly\nWork closely with the Individualized Support Team (IST) in assessing the Individual’s risks and assuring a plan\nProcess quality assurance, complaint, incident and change of status reports and notify the appropriate parties with the reports.\nPrepare, investigate and submit Incident Reports within 24 hours\nEnsure that each of the following are reviewed and agreed upon by the IST at each quarterly meeting:\nThe appropriateness of services and alignment with the Individual’s support needs to live their best life\nRisk assessments and current risk plans\nThe appropriateness of ISP goals\nProgress towards ISP/PCP goals\nThe appropriateness of current medication as well as compliance\nRecent and upcoming medical appointments\nHealth and safety of the Individual\nThe compliance of all files\nInternal Collaboration\nWork closely with LEL leadership to maximize supports provided to Individuals and their caretakers\nMonitor industry and local trends, advising leadership on service changes\nParticipate in company leadership meetings and events\n**Program Managers have calls/meetings approximately 3 times weekly outside of standard business hours.\n**Meeting with Individuals may require driving up to 4 hours\nSuccess Indicators\nMakes connection with every family at least once monthly\nVisit Individuals at least once quarterly\nCompletion of Incident Reports within 24 hours\nPrepare collaborative and thorough materials for quarterly meetings\nAdvocates for the needs of Individuals\nFriendly, supportive rapport with all Individuals, families, DSPs and members of the support team\nFile compliance\nProgram Manager must have a Bachelor’s degree in Human Services or related degree per state requirements\nFamiliarity with FSW, CIH, Pathways, Health & Wellness Medicaid Waivers.\nMinimum of two years full-time professional work experience preferred.\nSatisfies all requirements under applicable law.\nExperience and skills necessary to perform services listed above.\nStrong interpersonal and relationship-building skills.\nCustomer service oriented.\nA positive and pleasant attitude.\nAbility to work independently and be self-motivated.\nExceptional organizational skills and close attention to detail.\nStrong problem-solving skills.\nExcellent written and oral communication skills.\nStrong Microsoft Office experience (Outlook, Excel)\nDependable vehicle and valid Driver’s License\nPhysical Requirements:\nThe employee must:\nRegularly:\nSpeak\nHear (both in person and using a telephone)\nSit\nUse hands to manipulate, handle or feel\nReach with hands and arms\nLift and/or move up to five (5) pounds\nFrequently:\nStand, walk, stoop or kneel\nLift and/or move up to ten (10) pounds\nOccasionally:\nOperate a non-commercial automobile for distances up to 500 miles\nClimb and/or straddle\nLift and/or move up to 20 pounds\nWhile performing the duties of this job, the employee is regularly exposed to an office environment in which the noise level is usually low to moderate.
