Position SummaryTo support the Department of Discharge Services serving as care coordination resource staff by performing defined tasks that assist in the safe transition of patients to the next level of care.QualificationsHigh school or GED equivalentPost-secondary course work preferredKnowledge of medical terminology and managed care concepts preferredRefined written and verbal communication skillsProficient in computer skills and knowledge of Microsoft Office Suite programs Excellent organizational skills and attention to detailAbility to provide the highest level of customer serviceAbility to multi-task and prioritizeHigh level of maturity to manage confidential informationDress in professional manner in accordance with hospital policySkillsDetail oriented Communicate effectivelyAdaptability to changing work environment or needsResponsibilities & ExpectationsEnsure morning forms and reports are collected and distributedConveys Discharge information to those providing next level care such as primary care physicians, home care agencies and other post-acute care providersOrders supplies and maintains acceptable inventorySecures Post discharge PCP appointmentsSchedules post dc RN visitsAccurately records information and interventions in MIDAS and Cerner system to facilitate communication Secures, reviews and tracks patient specific data as defined by department and hospital initiatives such as readmissions, dual eligible, and PTHP volumesClarifies services and level of care with post-acute providers on hospital admissionAssists case management team with initial referrals, preparation of paperwork and transportation arrangements for next care provider along with calling, faxing, and copyingDocuments activities and actions in computerCommunicates concerns to other areas within the Hospital as appropriateFunctions as team member, working closely with staff to promote positive patient outcomesIdentifies opportunities for inter and intradepartmental operational improvementsProvides coverage for team membersParticipates in regular meeting forumsAll qualified applicants will receive consideration for employment without regard to race, color, religion, sex, age, national origin, disability, gender identity, sexual orientation or protected veteran status.Working Conditions6:30am – 5:00pm covered hours, M-F or hours as required by the job.
