Provides physical and behavioral health care coordination and promotes integrated medical and psychiatric care for ACT recipients with severe mental illness and/or substance use conditions.Duties and ResponsibilitiesProvides direct, face-to-face treatment and health screenings in the community.Guides team on preventative and critical health matters including nutrition.Administers injections and educates on medication adherence.Collaborates with psychiatrist and team on conditions and care delivery.Coordinates pharmacological services, labs, and referrals to providers.Implements wellness programs, health action plans, and risk assessments.Supports treatment planning and person-centered recovery goals.Provides staff training and education on health care integration.Completes all assessments, reports, and documentation on time.Qualifications And CompetenciesLicensed by NYS Education Department.3 years post-license experience treating individuals with mental illness.12 Years Wellness/health Program Experience Preferred.Strong organizational, communication, and critical thinking skills.Physical Demands & Work ConditionsTransport clients as needed.Travel to client homes and agency sites.Availability 8AM8PM with on-call support after hours.Direct in-person interaction required.Special Conditions of EmploymentBackground check/fingerprinting required.Valid drivers license and car insurance required.
