Wellness/Psychiatric Nurse

CenClear, a provider of mental health and substance use disorder (SUD) services, is seeking a dedicated and experienced Wellness/Psychiatric Nurse to join our team. The Wellness/Psychiatric Nurse will play a key role in training, program development, delivery, and monitoring of our services. Location 1 position based at our MH/SUD outpatient facility located in Saint Marys, PA. With other locations as needed. Compensation for time and mileage occurs based on the reimbursement policy. Hours This is a full-time position working 40 hours per week, 52 weeks per year (may include non-traditional hours). Qualifications Licensed Practical Nurse (LPN) Three years of experience in mental health services Maintain professional and confidential attitude. Effective communication and interpersonal skills. Attention to detail and accuracy in documentation. Flexibility to travel and work varied hours. Ability to gather and analyze health-related information. Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate Responsibilities Participate in all required CenClear, CCBHC and wellness/health related education and trainings Provide wellness coaching and related trainings to staff, individuals in services and community partners Promote a culture of health and wellness through ongoing participation in meetings and community outreach Collaborate and coordinate with healthcare providers for continuity of care Facilitate referrals, as needed Collect required data for outcomes measurement and contract requirements and document appropriately Submit all necessary administrative paperwork according to procedures Document all wellness coaching sessions and other wellness/integration activities Assist psychiatric prescribers in providing treatment to clients, collecting labs, reports, information and vitals for provider Follow up with clients per prescriber request Willingness to learn and adapt to new information and skills and engage in ongoing education, training and meetings. Self-motivation and ability to work independently as well as a team. Empathy and understanding in dealing with clients and families. Update client history and changes at each appointment. Maintain professionalism and confidentiality Complete paperwork accurately and in a timely manner. Attend and participate in monthly supervisions.
