Job Description
Position SummaryHamilton-Madison House's Supported Housing program provides long-term housing options for individuals with Serious and Persistent Mental Illness (SPMI), enabling them to live independently within the community. The Case Manager supports tenants in maintaining stable housing, improving independent living skills, and accessing community-based resources. This role emphasizes recovery-oriented, trauma-informed care and adherence to OMH guidelines and standards.ResponsibilitiesProvide ongoing case management services to a caseload of tenants in supportive housingConduct initial and ongoing assessments of tenant needs, strengths, and goalsDevelop, implement, and regularly update individualized service plans in collaboration with tenantsAssist tenants in obtaining and maintaining benefits (e.g., SSI/SSD, SNAP)Support linkage to mental health, medical, and substance use treatment providersConduct regular home visits to monitor housing stability, safety, and well-beingProvide crisis intervention and de-escalation as neededAdvocate on behalf of tenants with landlords, service providers, and community agenciesMaintain accurate, timely, and compliant documentation in accordance with OMH and agency standardsParticipate in team meetings, case conferences, and supervisionFacilitate life skills training, including budgeting, and daily living skillsPromote community integration and social engagementMinimum QualificationsBilingual proficiency in English and Korean preferredBachelor's degree in Social Work, Counseling/ Psychology or related field requiredAt least 1 years of experience working with individuals with serious mental illness or in supportive housing Knowledge of trauma-informed care principlesFamiliarity with OMH regulations and supportive housing models preferredStrong organizational, documentation, and communication skillsAbility to work both independently and as part of a multidisciplinary teamPay Range$26.50 to $28.00 per hour, commensurate with qualifications and experienceHoursMonday to Friday, 9:00AM to 5:00PM, 37.5 hours per weekBenefitsWe have a strong and vibrant work culture and offer excellent benefits including medical, dental, and vision insurance, 15 vacation days, 12 sick days, 3 personal days, and 12 paid agency-wide holidays.Hamilton-Madison House is an Equal Opportunity Employer
