Case Manager

JOB TITLE:  Case ManagerREPORTS TO: Clinical DirectorLOCATION: On Site Full TimeDESCRIPTION: Case management for substance use treatment program, working closely with the clinical staff to ensure that clients receive appropriate wraparound service in accordance with a client-specific service plan. Assess the functional level and program needs of each client and monitor service delivery.DESCRIPTION, DUTIES AND RESPONSIBILITY:Conduct treatment readiness public health sessions and refer participants to treatment service including substance use disorder treatment, mental health treatment and integrated treatment services.Coordinate client ground transportation with independent contractors, from area detoxes and to/from airports to Red Ribbon Recovery.Schedule initial meeting with client no more than 24 hours or next business day following client’s arrival at Red Ribbon Recovery for ongoing assessment, treatment planning and monitor compliance with treatment plan, adjusting treatment plan when necessary. Act as advocate and liaison for participants in securing access to community service and supportsComplete and maintain chart documentation following program standards and timelines Involve family, significant others, and EAPs and other ancillary treatment providers in service provision as and when neededLead psycho-education groups on addiction, recovery and coping skillsCase management for HIV+ participants, coordinate and communicate with MD’s and other healthcare providersAct as liaison with Employee Assistance Programs (EAP), Unions, referrers, and other appropriate departments or agenciesOther duties as assigned by the Clinical Director, Program Director or by the Executive DirectorMinimum Requirements:High School Diploma or GED and or 2 years’ experience with the target population.Preferred Requirements:Bachelor’s degree in human services or related field, and/or one plus years of providing services to the target population. Competencies:Excellent verbal and written communication skillsExcellent problem solving skillsAbility to experience and express empathyAbility to be non-judgmental and acceptingAbility to be flexible and adapt to changeHigh level of self-awarenessMulticultural competency
