Hourly Range: BR - BR (based on years of experience)Salary Range: BR,480 - BR,760 (based on 40-hour work week)25% Perm Placement FeeBR,000 Sign-On Bonus offered!Palliative Care:The Palliative Care Social Worker serves as an integral member of a self-directed inpatient interdisciplinary palliative care team. Provides comprehensive psychosocial assessments, serves as the mental health professional for the palliative care team, and provides appropriate social work interventions to meet the complex needs of customers and families/significant others in the hospital settings. The primary focus is to assist customers and families/caregivers in coping within the context of serious and life-limiting illness.What we'll expect from youAcute Units:Facilitates timely, efficient and appropriate discharge of customers and coordination with the next level of care, including, but not limited to: Transmitting of appropriate plan of care, Participating in and implementing discharge planning services for customers; Screening and coordinating new Nursing Home, Board & Care, Assisted Living Facilities, Group Homes, and Hospice facility referrals; Validating discharge criteria for customer and families; Following-up with customer, family and/or facility as indicated to ensure appropriate disposition and follow-up; Working with socially complex customers to identify barriers to discharge.Formulates, develops, and implements a comprehensive psychosocial treatment plan utilizing appropriate treatments and interventions. Interventions may include crisis intervention, brief individual, family and/or group therapies. Assists with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, domestic violence, rape, etc.Assessment and Psychosocial Evaluations: Assesses customers' and family's psychosocial risk factors through evaluation of prior functioning levels, appropriateness and adequacy of support systems, and reaction to illness and ability to cope; Provides intervention in child abuse/neglect/domestic violence, guardianship (temporary/permanent), foster care, adoption, mental health placement, and adult/elderly abuse.Intervenes with customers and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs.Offers supervision to Social Workers licensed at the LSW and LGSW levels.Serves as a resource regarding complex cases to interdisciplinary team, as well as within the department.Serves as a resource person and provides support and resource information related to palliative care treatment decisions and end-of-life issues.Family Conferences: Participates in customer/family care conferences; Records and appropriately manages customer and family concerns as appropriate.Collaboration with Team Members: Communicates and coordinates findings and care recommendations effectively and collaboratively between disciplines to achieve targeted outcomes.Financial: Maintains awareness of relevant payor requirements and partners with case managers, physicians and other healthcare professionals to address any issues related to denials and clinical appropriateness of admission and continued stay; Screens for financial needs and refers to appropriate team members and/or programs and communicates reimbursement information to customers and families.Reporting: Assists with adult and child protection reports.Serves as expert regarding county, state and federal resources including civil commitment and funding sources.Completes necessary assessment, community linkage and paper work for Civil Commitment process when indicated.Responds to customer/family concerns by working collaboratively with hospital customer representatives.Emergency Department:In addition to the Acute Unit accountabilities, the Emergency Department Social Worker shall:Serve as resource person and provide support to dying customers and their families.Offer support and resources to customers admitted to the trauma service, including help with adjustment to injury and community resourcesFacilitate placement for customers who do not meet medical necessity for hospitalization.Meet with high utilizers of the Hospital and Emergency Department to address barriers to getting care in the community.Meet with customers who are at high risk for readmission.Help divert customers from the Emergency Department to their primary care providerBuild community partnerships with clinics, shelters, and skilled nursing facilitiesAccepts other duties as assigned to promote the accomplishment of organization • Recognizes and complies with legal, regulatory, accrediting and procedural requirements related to area of responsibility.Understands and follows principles and standards as outlined in North Memorial's Corporate Code of Conduct.Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties.Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication.Understands and follows principles and standards as outlined in North Memorial's Corporate Code of Conduct.Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties.Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication.Recognizes and complies with legal, regulatory, accrediting, and procedural requirements related to area of responsibility.Understands and follows principles and standards as outlined in North Memorial's Corporate Code of Conduct.Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties.Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication.Requirement description : "By submitting your applicant to this need you are acknowledging that you have checked and have no inclination that your candidate has applied directly to this position. Any candidate that has applied directly is not eligible to be onboarded through an agency.***PLEASE SUBMIT CANDIDATES DATE OF BIRTH AT TIME OF SUBMISSION!***EducationMasters or Doctoral degree from an accredited school of social work requiredExperience, Knowledge, Skills, AbilitiesSocial workers who are required by clinical assignment to psychiatrically diagnosis or engage in the practice of psychotherapy shall have a minimum of 4000 hours of post-graduate clinical social mental health professional as defined by Minnesota statuteHospital social work experience preferred.Palliative Care only: Prior Palliative Care experience preferred.Psychiatric social work only: Experience with Serious Persistent Mental Illness (SPMI) population, mental health diagnoses.Knowledge of current case management principles, length of stay management, and/or transition/discharge planning highly preferred.Excellent verbal and written communication skills required.Knowledge of current social work principles, techniques and values.Licensure/Certification(s) MandatoryLicensure at the LICSW level in the state of Minnesota - NMHC Standard
