Telehealth Transitional Care Nurse Practitioner (Arizona - Must Have AZ License) Job Summary
We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality transitional care through telehealth visits. This role is ideal for a dedicated NP with strong skills in patient assessment, chronic disease management, and transitional care planning. The primary focus will be on ensuring smooth transitions for patients from hospital to home or skilled nursing facilities, reducing hospital readmissions, and improving overall patient outcomes.
Key Responsibilities
Conduct comprehensive patient assessments, including physical exams and medical history reviews.
Develop and implement personalized care plans for patients transitioning between care settings.
Manage acute and chronic conditions.
Provide telehealth consultations to expand patient access and enhance care coordination.
Collaborate with physicians, nurses, case managers, and other healthcare professionals to optimize patient outcomes.
Educate patients and families on medication adherence, disease prevention, and long-term health management.
Stay up-to-date with best practices in transitional and emergency medicine to ensure evidence-based care.
Requirements
Active and unrestricted Nurse Practitioner license in Arizona (Required).
Experience in transitional care, telehealth, or nursing home settings preferred.
Strong proficiency in medical-surgical nursing and emergency medicine.
Familiarity with ICD-9/ICD-10 coding for accurate billing and documentation.
Ability to work effectively in virtual healthcare settings.
Excellent communication skills, with a focus on patient education and engagement.
