Job Title: Clinic-based Case ManagerType: Full Time; Non-ExemptWork type: Onsite; Clinics and field sites across ChicagoPay Range: $22.00 - $26.00 per hourJob SummaryThe Case Manager plays a vital role in supporting clients by assessing needs, developing individualized service plans, and coordinating care across multiple systems. This position involves advocacy, assisting with aftercare services post inpatient hospitalization, and connecting clients to community-based resources to promote stability and well-being. The Case Manager provides comprehensive services for individuals navigating mental health challenges, substance use disorders, intellectual disabilities, and complex social service systems such as: SSDI, Housing, Medicaid, and Medicare.Key ResponsibilitiesClient Support & AdvocacyConduct IM CANS assessments every 6 months.Develop individualized treatment plans addressing social determinants of health, financial, mental/medical health, and legal needs.Review and update treatment plans every 90 days to reflect client progress or changes.Advocate for clients to access healthcare, financial, housing, and social services.Care CoordinationAssist clients with applications for IM-CANS, SSDI, LIHEAP, Childcare programs, and other assistance programs.Serve as a liaison with community agencies, healthcare providers both internal and external.Arrange transportation for clients to essential services and appointments as needed.Documentation & ComplianceMaintain accurate, confidential case files, documenting all client interactions, assessments, and follow-ups.Ensure compliance with organizational policies, state regulations, and legal requirements.Update IM CANS assessments every 6 months to ensure awareness of current client needs.Update treatment plans every 90 days to reflect evolving client needs.QualificationsEducation:Minimum of High school diploma required.Experience2 or more years of experience in case management, crisis intervention, healthcare, and/or social services.Experience with IM-CANS, SSDI applications, and acute mental illness preferred.Skills & AbilitiesStrong organizational skills and attention to detail.Excellent communication and interpersonal skills, with the ability to build rapport and advocate for clients effectively.Ability to handle sensitive and confidential information with professionalism and discretion.Strong electronic documentation skills required (EHRs, Microsoft Teams, Excel, and SharePoint).Ability to work independently and manage multiple tasks in a dynamic environment.Bilingual in Spanish preferred.Why Join Vivia Health?✔ Lead Impactful Work – Guide a gold-standard ACT program and drive real change in Chicago✔ Mission-Driven Team – Collaborate with passionate clinicians and visionary leaders✔ Professional Growth – Access CEUs, clinical supervision, and leadership mentorship✔ Supportive Culture – Inclusive, values-aligned environment where your voice mattersDetails✔ Location: Clinics and field sites across Chicago✔ Schedule: Full-time, Monday–Friday, 9 am - 5 pm✔ Employment Type: Full-Time, Non-ExemptBenefitsComprehensive medical, dental, vision, and life insuranceGenerous PTO and paid holidaysAnnual professional development stipend401(k) with employer matchSupportive, flexible workplace cultureReady to lead a team that changes lives every day?Join Vivia Health and help us build a more equitable, community-based mental health system—one person at a time.
