Nurse Practitioner With Hospice ExperienceAre you looking for a work place where you can make a genuine difference?Company Culture that feels supportive, genuine and appreciative of all?Anchor Health is committed to the communities of which we serve, the patients and families we have the honor of caring for and the EMPLOYEES who have chosen us as their work family.We are currently seeking a Nurse Practitioner with Hospice experience to join our Anchor Health team!QualificationsMaster's or doctoral degree in nursingCurrent NP License within the StateResponsibilitiesThe Nurse Practitioner (NP) is an experienced licensed professional who works in collaboration with the Hospice Medical Director/Physician to deliver hospice and/or palliative care to patients and familiesPerforms prescribed medical treatments, including patient examination, diagnoses, medication prescription, pain management and symptom control; conducts assessments and evaluations, and provides education and supportive care to hospice teams, patients and caregiversPerforms comprehensive patient examinations, evaluations, assessments and diagnosisAssesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action to control and manage patient symptoms and needsMakes adjustments to the already established plan of care and appropriate services, provides appropriate follow-up to all patientsDemonstrates flexibility in scheduling, and readily accepts new and emergency assignments as requestedPerforms timely face to face assessments with patientsDemonstrates positive interpersonal relation in communicating with physicians and co-workers to promote quality patient careCompletes all documentation in an accurate, legible and timely mannerProvides quality patient care consistent with standards of nursing practicesComplies with hospice policies and procedures, laws and regulations and standards of practice
