We are looking for a Social Worker (Case Manager II), who works under general supervision of CLTC Area Administrator. Serves as Case Manager Il. Assists in the implementation of a system of case management for Medicaid eligible applicants and enrolled participants. Assists other CLTC offices with essential job functions according to program needs. Trains workers and performs assigned quality assurance activities and compliance activities as directed. Travel to location of assigned cases necessary for this position. Effective time management skills and computer proficiency. Must be able to perform web-based software functions (key participant data, run reports, update documents, etc.).Knowledge of criteria for level of care determination. Knowledge of federal and state Medicaid regulations. Understanding and interest in working with the elderly and disabled. Ability to utilize and accept supervision and consultation. Ability to train new staff and contract providers. Ability to communicate effectively both verbally and in writing. Personal computer skills. Basic math skills.
