As a key member of our interdisciplinary team, you will play a vital role in providing exceptional care to our residents. Under the direction of our Licensed Social Worker, you will maintain accurate and timely documentation, communicate effectively with residents, families, and community agencies, and support each resident's right to self-determination.Responsibilities:Maintain accurate and timely documentationCommunicate effectively with the interdisciplinary team, residents, families, and community agenciesComplete interviews with residents as directed by the Social Worker or AdministratorDocument discharge planning and complete post-discharge calls to ensure a smooth transition for residentsSupport each resident's right to self-determination and document resident/patient refusal of services offeredAssist Social Worker team members with faxing documentation to agencies and/or other referral sourcesAttend and participate in staff meetings and other center meetings and sit on required committeesRequirements:High School Diploma (Experience in Long Term Care or Human Services field desirable)Two years of experience working directly with geriatric clients in a healthcare setting (desirable)Proficient in computer skills, including using word, excel, fax and email systemsPoint Click Care experience a plusPhysical demands: maximum lifting of approximately 10 pounds with occasional lifting of small objects; sitting, standing, walking, talking, and hearingGeneral Requirements:Must be able to execute the terms and conditions set forth in the Employee Handbook, including working in a safety-conscious manner, reporting safety violations or hazards, and interacting in a tactful, diplomatic, and humanistic manner with residents, families, visitors, and co-workersHow to Apply:If you are a compassionate and organized individual who is passionate about providing exceptional care to geriatric clients, please apply to this role.
