The ideal candidate will have experience in providing physical therapy services to patients in their homes. As a Home Health - Physical Therapist (PT), you will be responsible for assessing patients' needs, developing treatment plans, and providing therapy services to help patients regain their mobility and independence.Responsibilities:Conduct initial assessments of patients' physical abilities and develop treatment plansProvide therapy services to patients in their homesMonitor patients' progress and adjust treatment plans as neededCommunicate with patients' physicians and other healthcare professionals to coordinate careDocument patients' progress and maintain accurate recordsProvide education to patients and their families on how to manage their conditions and prevent further injuryRequirements:Current license to practice as a Physical Therapist in the state of TexasMinimum of 1 year of experience in home healthExcellent communication and interpersonal skillsAbility to work independently and as part of a teamStrong problem-solving and critical thinking skillsValid driver's license and reliable transportation
